 By submitting this form I allow Wesley Chapel's #1 Babysitting Services to take Pictures of   my child and i understand they could be used via advertisement on there business page or website flyers etc.

 

  I Agree i will not held Wesley Chapel's #1 Babysitting Services  accountable if for any reason we leave the agency and they continue to use those photos for advertisement purposes.

 

  i understand i have the right to not fill out this photo release form witch will with draw my child from being posted for via advertisement this form can only be filled out once per child.
